Creating a Lab Requisition Form with Medical Billing

You can create a lab requisition form replete with:

• The office name and address
• Requesting Physicians
• Patient Demographics including billing and insurance information
• The actual tests you wish (we set up the form with the most frequently used ones)
• Medicare waiver consent statement that the patient signs

Our staff (primarily the nurses) has instructions in the Knowledge Base (click here) for printing the requisitions.

You will also need to set this up so it prints on a laser. (click here) (This FILE IS 5.3+ MB so it will take some time).  Remember this is tedious but your staff will love you and the lab will like you for having the most up-to-date information for billing purposes.  Additionally, if you have a lab interface and the lab keys in the patient name, it is critically important that they get the correct spelling, chart number, etc.

the interface in Medical Billing for WINDOWS works the same way. You will just need
to tweak it a little to work with your printer.
